- Please bring 1-2 pairs of shoes.
- The payment for custom orthotics is due in full. The price for one pair is $560.00 (*assessment fee of $80.00 is waived). There is no direct billing.
- By appointment only. Insurance related documents provided upon pickup of orthotics.
